Information we collect
When you use our contact form, privacy-request form, call, text, or email us, we may collect information you choose to provide, such as your name, phone number, email address, state, insurance interests, and the contents of your message. Our hosting and form-service providers may also process limited technical information such as IP address, browser type, device information, and server logs for security and website operation.
How we use information
We use information you provide to respond to your request, communicate with you about insurance options and service, maintain appropriate business records, protect the website, and comply with applicable legal, regulatory, carrier, and insurance-licensing requirements.
Medicare and sensitive information
This website is not intended to collect Social Security numbers, Medicare beneficiary identifiers, banking information, or detailed medical information. Please do not submit those items through a website form. If sensitive information is needed for an application or enrollment, an appropriate secure process should be used.
Formspree
Website form submissions are processed by Formspree so that your request can be delivered to Porchstone Life Group and screened for spam. Information submitted through a Formspree-powered form is subject to Formspree's processing practices in addition to this policy.
Sharing and selling information
Porchstone Life Group does not sell personal information. Personal information may be shared with service providers that help operate the website, email, communications, compliance, or insurance-services workflow, and with insurance-related parties when necessary to provide a service you request or as permitted or required by law. Medicare beneficiary data is not intended to be shared with another third-party marketing organization for marketing or enrollment without the consent required by applicable law.
Contact permissions
If you submit a coverage-review request and check the consent box, you authorize Porchstone Life Group and Levi Samborski to contact you using the phone number or email address you provide regarding that request and relevant insurance options. Consent is not a condition of purchase. You may ask us to stop marketing communications at any time.
Cookies and analytics
The current website does not intentionally use advertising cookies, Meta Pixel, Google Analytics, or behavioral advertising trackers. Cloudflare may process technical and security information necessary to deliver and protect the website. If advertising or analytics tools are added later, this policy should be updated before they are enabled.
Data retention and security
Information is retained only as reasonably necessary for the purposes described above and to meet applicable legal, regulatory, carrier, recordkeeping, and business requirements. Reasonable administrative and technical safeguards are used, but no internet transmission or storage system can be guaranteed completely secure.
Your choices
You may request access to, correction of, or deletion of personal information Porchstone maintains about you, subject to applicable legal and recordkeeping obligations. Use the Privacy Request form, email levis@porchstonelifegroup.com, or call 313-802-4910.
Government affiliation
Porchstone Life Group is not affiliated with or endorsed by the U.S. government or the federal Medicare program.

This Privacy Policy may be updated as services, technology, or legal obligations change. The date above reflects the most recent revision.
